1. Make contact with Canine Specialty and request an application for a service dog.
A completed application includes the following:
a. Completed Program Application Form. b. Medical History Form completed by your physician or primary care specialist. c. Personal letter of reference from a friend, teacher, or someone other than a family member. d. Professional letter of reference from a therapist, social worker, teacher or any other professional with whom you have contact. e. $25.00 application fee.
The staff upon receipt will review your application. Those applicants who qualify for our program will be contacted to complete the second phase of the application process.
PHASE II
a. Will send a proposal of estimated fee for your service dog excluding $25 application fee. b. Fill out pre-interview form c. Set up interview by phone or video. d. Fill out personality profiles. e. Biography and picture. PHASE III
a. Acceptance of proposal by client sent to training center. (The proposal includes detailed information regarding each payment and what it covers.) b. Initial training fee of up to 50% of cost of dog. c. Training center selects several service dog candidates and begins training process. d. Estimated date of placement is given.
PLACEMENT
a. $500 team training fee due at beginning of team training. b. TEAM TRAINING c. Final fee due upon acceptance of service dog. d. Ownership of service dog transferred to recipient.
